Scabies is a contagious skin infestation caused by a tiny mite called Sarcoptes scabiei. It commonly causes intense itching, especially at night, along with small red bumps or a pimple-like rash. Scabies can affect people of any age and can spread between household members through prolonged close skin-to-skin contact.
Important: Scabies treatment usually requires a specific scabicide medicine. This article explains commonly used medicines and general treatment principles. It does not provide individualized treatment or replace a doctor’s examination.
What Is Scabies?

Scabies occurs when microscopic mites burrow into the upper layer of the skin and lay eggs. The body’s immune response to the mites and their waste causes itching and rash.

Common Symptoms of Scabies
- Severe itching, often worse at night
- Small red bumps or pimple-like rash
- Thin burrow-like lines on the skin
- Rash between the fingers
- Itching around wrists and elbows
- Rash around waist, armpits and buttocks
- In some cases, genital or nipple-area involvement
Scabies Medicines – What Medicines Are Used?

Scabies is treated with medicines called scabicides, which are designed to kill scabies mites. Commonly used treatments include:
- Permethrin 5% cream
- Ivermectin tablets
- Benzyl benzoate preparations
- Sulfur preparations
- Crotamiton in some situations
The choice depends on age, pregnancy status, severity, medical history, availability and the clinician’s assessment. WHO lists topical permethrin, benzyl benzoate and sulfur preparations among treatment options, and oral ivermectin is also effective in appropriate patients.
1. Permethrin 5% Cream
Permethrin 5% cream is one of the commonly recommended treatments for uncomplicated scabies. CDC guidance describes application to the body from the neck downward, followed by washing it off after approximately 8–14 hours.
Permethrin 5% cream
Important Application Principle
Scabies treatment is not simply about putting cream on the visible rash. The medicine generally needs to be applied to the appropriate body areas as instructed, because mites may be present in areas without obvious rash.
Children
Young children may require different application instructions. CDC recommends permethrin for infants and young children, while oral ivermectin safety has not been established in children weighing under 15 kg.
2. Ivermectin Tablets
Ivermectin is an oral medicine that may be used for scabies in appropriate patients. CDC guidance lists an oral dose of 200 micrograms/kg, with a second dose 14 days later because ivermectin has limited activity against mite eggs.
Important
Do not self-dose ivermectin.
The correct amount depends on body weight and medical circumstances.
WHO notes that the safety of ivermectin has not been established in pregnancy or in children below 15 kg.
3. Benzyl Benzoate
Benzyl benzoate is another topical scabicide used in some settings. WHO lists 10–25% benzyl benzoate preparations among treatment options.
Why Product Strength Matters
A benzyl benzoate product may have a different concentration depending on the formulation and intended age group. Therefore, patients should follow the specific product instructions or medical prescription.
4. Sulfur Ointment
Sulfur preparations are another option listed by WHO, including 5–10% sulfur ointment.
5. Crotamiton
Crotamiton may sometimes be used for itching associated with scabies, depending on the clinical situation and local availability.
Scabies Medicine Dosage Principles
Scabies medicines do not all have the same dose or application schedule.
Permethrin
A common CDC regimen is:
Permethrin 5% cream → apply as directed → leave for 8–14 hours → wash off.
Ivermectin
CDC lists:
200 micrograms/kg orally → repeat after 14 days.
However, this is not a personal prescription. The suitability of oral ivermectin depends on age, body weight, pregnancy status, other medicines and medical conditions.
Why Does Scabies Sometimes Come Back?
Treatment failure does not always mean that the medicine did not work.
Possible reasons include:
- Incorrect application of cream
- Not treating close household contacts
- Reinfection from another person
- Not following the treatment schedule
- Persistent allergic reaction after mites have been killed
- Severe/crusted scabies requiring specialist management
CDC notes that itching can continue for less than two weeks or sometimes longer after successful treatment because the body’s allergic reaction to mites can persist.
Treat Close Household Contacts
One of the most important principles of scabies treatment is simultaneous management of affected household contacts.
People can have early scabies without symptoms and still spread it. WHO recommends treating the whole household at the same time in appropriate situations.
Clothes, Bedding and Towels
Scabies mites generally do not survive for long away from human skin. CDC recommends washing bedding, clothing and towels using hot water and a hot dryer when possible. Items that cannot be washed can be sealed in a plastic bag for at least 72 hours.
Scabies Medicine Side Effects
Different medicines can cause different side effects.
Permethrin Side Effects
Possible reactions include:
- Burning or stinging
- Redness
- Skin irritation
- Temporary increase in itching
Ivermectin Side Effects
Possible side effects can include:
- Dizziness
- Nausea
- Headache
- Weakness
- Gastrointestinal discomfort
If a severe or unusual reaction occurs, medical attention should be sought.
Precautions Before Using Scabies Medicines
1. Pregnancy
Pregnant women should not self-medicate for scabies. WHO notes that ivermectin safety in pregnancy has not been established, while topical treatments such as permethrin are commonly preferred in appropriate circumstances.
2. Breastfeeding
Breastfeeding mothers should discuss treatment options with a healthcare professional.
3. Young Children
Children require age-appropriate treatment. Oral ivermectin should not be assumed to be suitable for children under 15 kg.
4. Crusted Scabies
Crusted scabies is a more severe form and can spread more easily. It may require combination treatment and specialist care.
Scabies Medicine Drug Interactions
Drug interactions are particularly important with oral ivermectin.
CDC notes that ivermectin treatment choices can be affected by interactions with medicines including azithromycin, trimethoprim/sulfamethoxazole and cetirizine.
Before Taking Ivermectin, Tell Your Doctor About:
- Prescription medicines
- Antibiotics
- Allergy medicines
- Other antiparasitic medicines
- Liver problems
- Pregnancy or breastfeeding
- Other chronic medical conditions
When Should You See a Doctor?
Seek medical evaluation if:
- The diagnosis is uncertain
- Itching is severe
- New burrows continue to appear
- Symptoms continue despite correct treatment
- Several family members have similar symptoms
- There are signs of skin infection
- The patient is an infant
- The patient is pregnant
- The patient has a weakened immune system
- There are extensive crusted skin lesions
